I agree to the following statement.

We, the team, assume full responsibility for the players' medical expenses and well being and waive all rights against the Queen City Classic and
the Spearfish Spartan Athletic Booster Club and all others associated with this tournament from all claims caused by injuries which may be sustained by our people who are participating in this tournament. I understand all conditions and verify that the above information is correct.

TOURNAMENT CANCELATION POLICY:(Required)

I have read and agree to the following policy:

25% of the entry fee will be retained in the event that a team drops out after the registration
deadline or if the tournament is cancelled due to inclement weather or any other situation.

Should the tournament be postponed, half of the entry fee will be retained for those teams that can not participate on the make up date for the tournament.

$25 of entry fee will be retained if a team request a refund after registering for the tournament and prior to the deadline.

50% of the entry fee will be retained shall a team request a refund after the deadline but prior to brackets being prepared.

100 % of the entry fee will be retained for teams who cancel after the brackets have been prepared and posted and for teams who do not show up to participate.
